Student-Athlete Insurance Information
Instructions for Completing Athletic Training Forms
- Complete all forms thoroughly.
- You must print the forms (using Adobe Acrobat Reader) and write or type the information in (if handwritten, please make sure that it is legible).
- Once you have completed the forms, check to make sure that all information is completed correctly.
- There are sections of the forms that need to be signed by student-athlete, parent(s)/guardian(s), or both. Make sure that these are correctly completed before submitting them.
- Please send a copy of the front and back of the student-athlete's insurance card.
- Once all of the forms are completed and signed please send them to the following address:

- Please make sure that these forms are returned to the Head Athletic Trainer as soon as possible. FAILURE TO DO SO WILL RESULT IN THE STUDENT-ATHLETE BEING UNABLE TO PARTICIPATE IN ANY FORM OF ATHLETICS (PRACTICES, CONDITIONING, ETC.) UNTIL ALL PAPERWORK IS COMPLETED.
- Also note that it is not necessary to have a physical before arriving on campus. A pre-participation exam will be given upon your arrival at Cameron University free of charge. Your coach will notify you of your scheduled time. NOTE: IF YOU DO NOT SHOW UP FOR YOUR SCHEDULED PHYSICAL, YOU WILL BE FINANCIALLY RESPONSIBLE FOR PROVIDING THE REQUIRED DOCUMENTATION.
